Category: Surgical Therapy: Parkinson's Disease
Objective: We aimed to clarify long-term changes in activities of daily living in patients with Parkinson’s disease (PD) who underwent deep brain stimulation (DBS).
Background: It is well known that DBS dramatically alleviates motor complications in PD. However, long-term changes in activities of daily living are not well elucidated.
Method: We retrospectively reviewed seventeen patients with PD (mean age 65.18±1.42 years, mean disease duration 11.94±0.57 years) who underwent subthalamic nucleus (STN)- DBS and completed pre-operative and post-operative (one year and three years after surgery) evaluations. Activities of daily living (ADL) and motor functions were evaluated by Unified Parkinson’s disease Rating Scale (UPDRS) Parts 2 and Parts 3, respectively.
Results: The score of UPDRS-Parts 2 during on phase was significantly increased three years after surgery compared to baseline (6.82±1.32 → 13.29±1.93, p=0.011), whereas the score of UPDRS-Parts 3 during off phase was significantly decreased three years after surgery compared to baseline (39.24±2.91 → 26.71±3.12, p=0.01). Of the seventeen patients, the score of UPDRS-Parts 2 during the on phase increased more than six in eight patients (ADL deteriorated group). The differences in the score of UPDRS-Parts 3 during the off phase between baseline and three years after surgery were significantly reduced in the ADL deteriorated group compared to the no ADL deteriorated group.
Conclusion: Some patients with Parkinson’s disease showed deteriorations of ADL within three years after DBS.
