Sleep disordered breathing in patients of Parkinson’s disease with REM sleep behavior disorder

Objective: This prospective study was done to study the frequency and pattern of sleep disordered breathing (SDB) in patients with PD with RBD.

Background: Parkinson’s disease (PD) patients can have REM sleep behavior disorder (RBD) which is a parasomnia characterized by complex motor activity emerging from REM sleep. There is relatively low prevalence of SDB in patients with PD however there is less data on PD with RBD.

Methods: The study was conducted in department of neurology, NIMHANS, Bangalore and included 27 PD patients with probable RBD (questionnaire based) and 18 controls. Patients underwent overnight video polysomnography. The sleep scoring was done as per the standard criteria. Sleep apneas were recorded as per the American academy of sleep medicine (AASM) rules. Statistical analysis was done using R software. Comparison was done using independent student ‘t’ test and chi-square test.

Results: The mean age of the patients was 61.15±8.4 years with a range of 43-78 years. Women constituted 14.8% of the study group. The mean age at disease onset was 54.7±10.0 years and mean RBD duration was 2.8±2.6 years. The total sleep time was 4.3±1.7 hours with sleep efficiency of 53.8±21.0%. Most of the patients spent their sleep in stage N2 (41.4%). 22 patients had polysomnography confirmed RBD. About 51.8% patients had obstructive sleep apnea, 33.3% had central sleep apnea and 11.1% had mixed apnea. The mean number of OSA episodes was 4.5±7.6, central sleep apnoea was 0.9±0.7 and mixed apnoea was 0.4±1.7. The minimum oxygen saturation was 85.3±7.4% and the maximum oxygen saturation was 99.8±0.4%. The desaturation index was 5.6±17.0. The mean value of apnea-hypopnea index was 4.9±11.2.

Conclusions: Patients of PD with RBD have relatively higher occurrence of sleep apnoeas. This has implications on assessment and management of these patients.
