Objective: Presenting the profile of the position of specialized nurse coordinator in a unit caring for implanted therapeutic systems (stimulators and pumps) in functional neurosurgery. Presenting the initial experience (20/10/2015).
Background: A growing number of neurological disorder patients have access to continuous electrical neuromodulation (DBS). Our Centre has decided to create a position of nurse coordinator to improve the quality of patient follow-up. This position is now indispensable for care during hospitalization or by keeping a telephone link after their return home. He participates in the care of implanted systems. The "resistant cerebral pathology" unit monitors 1,362 patients including 490 patients (DBS) and 35 patients (intrathecal infusion pumps).
Methods: The participation of specialized paramedical staff was described.
Results: – Functions:
• Informing and educating patients. Pre-surgical information, participation in the process of informed consent. Preparation and transmission of the information document. Training – patients in the recharge protocol. Being the intermediary between patients and the medical team, through a telephone hotline and voice-mail (special number). • Formalization of the care protocol, control of the quality of implementation, coordination of multidisciplinary follow-up. • Collaboration with the medical secretaries in the programming. • Collaboration with the units: paramedical and medical staff. • Technical gestures:
Monitoring of implanted stimulators and parameters in liaison with the doctor. Filling the baclofen pumps. – Experience: this position, established in 2010, has demonstrated its effectiveness. In 2015, I am present in consultation one half-day per week. I provide three half-days of telephone on-call and a permanent e-mail link given to the patient.
Conclusions: The care of disabled patients, requiring the prolonged use of specialized technologies, is difficult to insert into the ‘normal’ operation of hospitals (heaviness of poly-handicaps, medical hyper-specialization, additional costs). The participation of specialized paramedical staff facilitates care and makes it possible to take a greater number of patients. The nurse-coordinator now provides an indispensable function in this complex mechanism.
