Objective: To study affective disorders and their impact on the quality of life in patients with PD and chronic pain syndrome.
Background: Mechanisms of the development of pain in PD and its relationship with other disease manifestations are poorly understood. All this makes relevant a comprehensive clinical analysis of chronic pain syndromes in patients with PD.
Method: 99 patients (ages 64,7±8,2) with PD were examined. Two groups: 1st (63 pers.) – patients with chronic pain syndrome, 2nd (36 pers.) – patients without pain – were formed. Assessment of pain (VAS, pressalgometry), affective disorders (Beck Depression Inventory (BDI), obsessive-compulsive syndrome scale), quality of life (EQ-5D and scale PDQ-39) were performed.
Results: Depressive symptoms were observed in 45 (71,4%) patients with pain and in 12 (33,3%) patients without pain (p<0,01). The average BDI score in patients with pain was 16,2±8,4, whereas in patients without pain was 13,8±7,1. Mild symptoms of depression were determined in 21 (33,3%) patients with pain and in 8 (22,2%) patients without pain; moderate – in 13 (20,6%) and in 8 (22,2%) patients; severe – in 28 (44,4%) and in 8 (22,2%) patients respectively (p<0,05). The severity of pain, correlated with the severity of depressive symptoms (R=0,52, p<0,001). Obsessive-compulsive syndrome in patients with pain was severer (52,9±22,7) than in patients without pain (42,8±18,5, p<0,05). A decrease in vibration sensitivity was observed in 82,5% of patients in the 1st group, and in 41,7% of patients without pain (p<0,01). The assessment of the quality of life also depended on the severity of depressive symptoms (R=-0,54; p<0,001).
Conclusion: Pain, along with motor and affective disorders, is a factor in reducing the quality of life of patients with PD. The decrease in vibration sensitivity indicates the presence of hidden sensory disorders in patients with PD, which can contribute to the formation of chronic pain syndrome.
