Objective: To determine whether RLS could be associated with a specific psycho-behavioral profile and ICDs in Parkinson’s disease (PD).
Background: Impulse Control Disorders (ICDs) have been described in patients with idiopathic Restless Legs Syndrome (RLS) under dopaminergic therapy (1). Untreated patients with idiopathic RLS have also displayed increased impulsive choices compared to controls (2).
Methods: Seventy-one consecutive PD patients were screened for the presence of RLS in a cross-sectional study. Sleep features were evaluated during one video-polysomnography. The frequency of ICDs, according to standard criteria, together with a broad range of psycho-behavioral features using the Ardouin Scale of Behavior in PD, were compared in patients with RLS (PD-RLS, n=28) versus without RLS (PD-noRLS, n=43).
Results: PD patients with RLS reported significantly more ICDs than those without RLS (53% versus 27.9%, p=0.02), especially compulsive eating disorders, and a different psycho-behavioral profile with more hyperdopaminergic behaviors. The groups were comparable for age, sex ratio, occurrence of REM Sleep Behavior Disorders, and disease severity. There was no between group difference for total levodopa equivalent dose (LED) and Dopamine agonists LED. However, the durations of both disease and dopaminergic treatment were longer in the RLS group. Propensity score analyses controlling for age, gender, dose and duration of treatment, showed that RLS was an independent predictor of ICDs in PD (OR=3.85[2.36;6.50]).
Conclusions: ICDs and hyperdopaminergic behaviors are more frequently reported in PD-RLS compared to PD-no RLS, regardless of dopatherapy dose and duration, suggesting that RLS per se could be a risk factor for impulsive behaviors in PD.
