Predictors of transfer to facility or nursing home after discharge of inpatients with Parkinson’s disease: A Japanese single center retrospective study

Objective: To investigate the reasons for hospital admissions for Parkinson’s disease (PD) patients and to determine the predictors of transfer to facility or nursing home after discharge.

Background: During their long clinical course, patients with PD may require hospitalization due to the disease itself or a combination of other diseases. Once hospitalized, their prognosis is often poor and sometimes they cannot return to their original life. However, there are only few studies that have investigated reasons for hospitalization, and reasons for PD inpatients being admitted next to transfer to another hospital or nursing home.

Method: A single center retrospective study was done. Subjects were identified from discharge records listing diagnoses of PD from the administrative inpatient data set of Himeji Central Hospital, a private hospital in a medium-sized city in Japan from January 2017 to December 2018. All electronic medical records of PD inpatients were consecutively reviewed to collect patient data (demographic data, disease duration, reasons for admission, type of admission (planned or urgent) and outcomes).

Results: During the study period, 229 PD patients were hospitalized. 175 patients were discharged from the general ward, 12 from the convalescent rehabilitation ward, and 42 from the community-based ward. The causes of hospitalization were as follows. PD symptoms worsening 48, DBS-related 33, pneumonia 16, fracture 14, LSVT rehabilitation program 13, xylokine administration to the abdominal muscle 12; cerebrovascular disease 11; PEG related 6, ICIG related 6,  pyelonephritis 3, and others 26. The hospitalization source and discharge destination are followings: admission from home and discharge to home 180, admission from home and discharge to nursing home 12, admission from nursing home and discharge to nursing home 13, admission from other hospital and discharge to home 13 and eleven patients are died.

Conclusion: In this survey, more than 100 PD patients were hospitalized annually in our hospital. The most common cause of hospitalization was PD symptoms worsening, followed by pneumonia and fractures. Only 6.3% of patients admitted from home could not return home. This is because our hospital has a convalescent rehabilitation ward and a community-based ward.
