Objective: As the previous studies indicated that other factors including REM sleep behavior disorder (RBD) and olfactory dysfunction were also associated with cognitive decline and gray matter atrophy in PD, we examined whether OH and RBD interacts on brain atrophy in PD.
Background: Orthostatic hypotension (OH) is frequent autonomic nervous system dysfunction in Parkinson’s disease (PD) and is strongly associated with cognitive impairment although the underlying pathophysiology remains unclear. Recent study demonstrated that early OH, but not its symptom severity, increases the risk of cognitive impairment in patients with PD.
Method: This was a retrospective cohort study of patients with PD who were admitted to the Jikei University Hospital. Seventy-eight de novo PD patients were enrolled in this study. All subjects were underwent head up tilt test. T1-weighted images were preprocessed using FreeSurfer. Cortical thicknesses were extracted were resampled and smoothed. Besides analyses of cortical thickness, we performed analyses of subcortical gray matter structures which were extracted based on the aseg atlas. The presence of RBD status was assessed using the Japanese version of RBD Screening Questionnaire (RBDSQ-J).
Results: Whole-brain analyses of cortical thickness revealed that these was no effect of the presence of OH or RBD after Monte Carlo corrections. However, in the OH+ group, these was a effect of RBDSQ-J on bilateral thalamus, putamen, hippocampus, and amygdala after false discovery rate < 0.05 corrections, whereas no significant relationship was observed in the OH- group.
Conclusion: Subcortical atrophy in PD patients with RBD was reported in the previous study (Oltra J, et al. 2022). Orthostatic hypotension exacerbates gray matter atrophy related to REM sleep behavior disorder in de novo Parkinson’s disease.
