Objective: To investigate swallowing and its coordination with respiration in patients with Parkinson’s disease.
Background: Patients with Parkinson’s disease usually present with speech and swallowing dysfunction, both of which are associated with a poor quality of life.
Methods: This prospective cohort study was performed at a tertiary referral medical center in Taiwan, and included 37 controls and 42 patients with early-stage Parkinson’s disease. A non-invasive system was used to evaluate oropharyngeal swallowing parameters and the coordination with respiration during swallowing. Five different water boluses (maximum 20 ml) were swallowed three times, and the data were recorded and analyzed.
Results: There were differences in oropharyngeal swallowing parameters between the patients and controls, including a longer total excursion duration and excursion time in the patients, but no difference in the length of swallowing respiratory pause. The rates of non-expiratory/expiratory pre- and post-swallowing respiratory phase patterns were higher in the patients than in the controls (P < 0.001). The rates of piecemeal deglutition when swallowing 10-ml and 20-ml water boluses and overall were also significantly higher in the patients (all P < 0.001).
Conclusions: Oropharyngeal swallowing and its coordination with respiration were affected in the patients with Parkinson’s disease, and these effects could be detected using a non-invasive method. The results of this study may serve as a baseline for further research into new treatment regimens and to improve the management of swallowing in patients with Parkinson’s disease.
