Session Title: Parkinson's Disease: Neuroimaging And Neurophysiology

Objective: To study mineralization pattern of the deep gray matter by susceptibility-weighted (SWI) magnetic resonance imaging to differentiate Parkinson’s disease and atypical parkinsonian disorders like Progressive Supranuclear Palsy (PSP), Multisystem Atrophy (MSA), corticobasal denegation (CBD), etc.
Background: Idiopathic Parkinson’s disease (IPD) is most common (80-85 %) and has good response to Therapy and has to be differentiated from atypical parkinsonism disorders (APD), i.e., progressive supranuclear palsy (PSP), multiple system atrophy (MSA) and Cortcobasl degeneration (CBD) for prognostication and optimization of treatment.
Methods: 30 subjects were enrolled for the study out of which 10 were in control group and 20 patients presented with features of Parkinsonism. MR imaging examinations were performed with a 18-channel 3.0 T whole body imager. (Siemens). In addition to routine MR imaging of brain including T1WI, T2WI and FLAIR sequences were done, SWI were done. The hypointensity of poster lateral putamen, red nucleus, substantia nigra, and dentate nucleus in all groups were measured in comparison with SI of the CSF and vein of Galen.
Results: In the conventional MRI presence of midbrain atrophy, superior cerebellar peduncle signal changes and atrophy in patients of PSP differentiate them from patients of MSA-P and PD. The increased hypointensity scores of red nucleus on SWI in patients of PSP differentiate it from PD and MSA-P. The increased hypointensity score of substantia nigra in patients of PD support the site of pathology and differentiate them from atypical Parkinsonism patients.
Conclusions: The routine MRI techniques available to differentiate PD and APD patients can help the clinician in differentiating these conditions only in advanced stages of the disease, we observed that SWI is a simple and advanced modality, which can help better in differentiating PD from APD at an early stage.
