Session Title: Parkinson's Disease: Non-Motor Symptoms

Objective: To determine the frequency of dysautomic disorders and structural cardiac abnormalities in patients with early PD.
Background: Parkinson´s disease (PD) is a neurodegenerative disease characterized by motor and non-motor symptoms. Among the later, disorders of the autonomic nervous system such as orthostatic hypotension (OH) are common. Other autonomic abnormalities (AA) such as nocturnal hypertension (NH) and the loss of nocturnal blood pressure dipping (LNBPD) have been associated to other cardiovascular abnormalities (CA) and mortality. The most common CA are left ventricular hypertrophy (LVH), cardiac ischemia, heart failure and arrhythmias.
Methods: A cross-sectional study was carried out. Twenty patients with early PD (<5 years) were included. A structured questionnaire was applied. MDS-UPDRS part 3 and Höehn and Yahr were aplied. Autonomic testing included table tilt test, Valsalva index, and OH assessment. Cardiac evaluation included echocardiogram. In addition, a 24-hour ambulatory blood pressure monitoring was performed.
Results: A total of 11 males and 9 females were included. The mean age was 61.4±6.1 and a disease duration of 4.2±0.9 The mean MDS-UPDRS part 3 was 17±9. LVH was found in 5 patients, NH in 11 patients, and LNBPD in 13 patients. On the other hand, heart variability was abnormalin all cases. OH was found in 12 cases. In most cases, no corresponding symptoms were reported.
Conclusions: Both cardiac and dysautonomic disturbances are frequent in early PD patients; although mostly subclinical; it is therefore important to look for these disroders through proper testing in order to impact their quality of life and morbi- mortality.
