Objective: Dystonia is one of the most debilitating movement disorders. Pallidal DBS has been revolutionary for the management of medically refractory dystonia. Although DBS of GPi has been widely performed worldwide for medically refractory dystonias, the number of patients undergoing pallidal DBS is relatively scarce in India.
Background:
Aim of this article is to share our experience on pallidal DBS in patients with dystonia in a tertiary care neuroscience hospital.
Methods:
A chart review of patients who had undergone pallidal DBS for dystonia (till 2016) in the Department of Neurosurgery and Neurology at National Institute of Mental Health and Neurosciences (NIMHANS), Bangalore was done. Severity of dystonia was assessed by the Burke- Fahn-Marsden dystonia rating (BFMDRS) scale before surgery as well as during the follow up visits inorder to monitor the effect of DBS.
Results:
We observed significant improvement in the total score of the BFMDRS as well as the movement and disability components during follow up at 3rd, 6th and 12th month after DBS in all the patients. Most significant improvement was obserbed in the axial component of the dystonia rating scale. Other than infection in one patient, severe intra-or post-operative complications were not observed.
Conclusions:
Pallidal DBS significantly improves the severity of dystonia and quality of life. Surgical or neurological complications usually rare. Long term follow-up studies may provide further insights into the efficacy of pallidal DBS for dystonia.
