Objective: To demonstrate cognitive outcome after subthalamic deep brain stimulation(DBS) in the patients with Parkinson’s disease(PD) in Taiwan.
Background: Deep brain stimulation of the subthalamic nucleus (STN) is the most important innovation for treatment of advanced Parkinson’s disease. In addition to the improvement of motor symptoms of PD patients, STN DBS also affects the cognitive performance, including verbal fluency, verbal memory and executive function.
Methods: Within one year follow-up, comprehensive cognitive battery, unified Parkinson’s disease rating scale (UPDRS) and levodopa equivalent daily dose (LEDD) were assessed among 17 PD subjects receiving STN DBS.
Results: The mean age was 60.1 (SD 10.4) years and the mean disease duration was 11.4 (SD 3.9) years. A decline and trend of decline in semantic verbal fluency (in Fruit and Transportation category) were detected after one year STN DBS implantation. The decrease of dosage of dopamine replacement therapies achieved clinical significance after adjusting several clinical parameters with logistic regression.
Conclusions: Verbal fluency decline was detected in our patients after one year STN DBS therapy. A post-operative reduction in dopamine replacement therapies might play a role in the decline in verbal fluency after STN DBS.
