Results: Stiff-limb syndrome (SLS) is an uncommon variant within the stiff-person spectrum of disorders (SPSD) and is characterized by progressive stiffness and pain of a limb, usually the leg. It can be a paraneoplastic process, and in some instances the neurologic symptoms precede the diagnosis of the associated malignancy. Anti-amphiphysin antibody/breast cancer-associated SLS is a relatively rare phenomenon (1-5% of those with SPSD), with varying outcomes. We describe the case of a 65 year-old woman who subacutely developed severe left arm stiffness and pain, with myoclonic jerks. Upon evaluation, a localized left-sided breast cancer (invasive ductal carcinoma) was detected. Serum paraneoplastic screening was strongly positive for anti-amphiphysin antibody. The patient’s SLS improved substantially with a combination of cancer treatment (mastectomy), intravenous immunoglobulin, and physiotherapy/exercise. We also performed a systematic review of the literature on the management and outcomes of patients with anti-amphiphysin antibody/breast cancer-associated SPSD. Of the 13 patients reported, it appeared that neurological outcome was favourable in the majority of patients, whether or not immunotherapy was administered, provided the underlying cancer was treated.
