Objective: To examine baseline prevalence and longitudinal change in non-motor symptoms (NMS) in a cohort of de novo (at baseline) Parkinson’s disease (PD) patients compared with healthy controls (HC).
Background: NMS are increasingly recognized as the leading cause of disability and quality of life impairment in PD. There are still limited data on the prevalence, longitudinal evolution and clinicobiological correlations of NMS in early PD.
Methods: Parkinson’s Progression Markers Initiative (PPMI) is a longitudinal ongoing case-control study of de novo, untreated (at enrolment) PD participants and HC of similar age and sex. Participants complete annually a wide range of motor and NMS measures, including biofluids and imaging biomarkers. For these analyses NMS was rated with the MDS-UPDRS Part I total score and subscores for cognition (Q 1.1-1.6), sleep (Q1.7-1.8) autonomic dysfunction (Q1.10-1.12) as well as other validated NMS scales available in the PPMI dataset.
Results: There were 423 PD subjects and 196 HC at baseline. Longitudinal data at the time of analysis were available for 397 PD subjects at one year and 380 at 2 years. MDS-UPDRS Part I total mean (SD) scores increased from baseline 5.6 (4.1) to 7.7 (5.0) at year 2 in PD subjects (p<0.001) vs. from 2.9 (3.0) to 3.2 (3.0) in HC (p=0.38), with a significant difference between the groups (p<0.001). There was a significant increase over time in every MDS-UPDRS Part I subscore, as well scores of the GDS, SCOPA-AUT and QUIP, in PD subjects and no significant change in HC.
Conclusions: There is a significantly higher baseline burden and longitudinal increase of NMS disability in early PD compared with the general population. The biological correlates of NMS and impact on progression of PD disability will be assessed over time.
