Objective: We analyse an extension type of Parkinson’s disease.
Background: At the International Congress of the Movement Disorder Society last year, we reviewed a patient called Bachère, presented by Charcot in 1888 with an extension type of
Parkinson’s disease (PD). Here, on the basis of contemporary knowledge, we compare the two types: classic PD and extension PD.
Method: Using images of patients, we measured the flexion angles characteristic of both types.
Results: The neck and trunk as well as the upper limbs were more extended in the Bachère type.
Conclusion: These results suggest that the concept of Parkinson’s disease as understood in 1888 was broader in scope, and that multiple sclerosis was a possible deferential
diagnosis.
