Objective: The aim of this study was to exam relationships between REM sleep muscle atonia (RSWA), dream-enactment behavior(DEB) and clinical characteristics in Parkinson’s disease (PD) patients.
Background: Parallels between PD and REM sleep behavior disorder (RBD) have been reported for many years. According to ICSD-II, RBD had lost of RSWA and DEB.
Methods: We conducted overnight polysomnography studies in 145 PD patients. Motor behaviors and/or vocalizations in REM sleep with a purposeful component other than comfort moves were identified as DEB. Tonic chin EMG density≥30% or phasic ≥15% were identified as high RSWA. All PD patients were categorized into 4 groups. Group A is the patients who had DEB and RSWA.Group B is the patients who only had DEB ,while group C only had RSWA. Group D had neither DEB nor RSWA. These datas compared among study groups, and correlated with clinical characteristics.
Results: We found that group A,B,C were older. After adjusted for age, the patients of group A,C were higher H-Y stages, higher scores of UPDRS II than group D (2.5±0.7, 2.5±0.8, 1.9±0.6,P<0.005; 25.2±10.7, 22.3±9.2, 19.0±10.6, P<0.005). Group A,B,C had more levodopa-equivalent daily dose(LED) than group D (480.7±377.3mg/d, 389.2±329.4mg/d, 461.6±312.6mg/d, 230.3±216.4mg/d, P<0.005). After adjusted for age, the tonic and phasic EMG activity were observed to be associated with PD duration, H-Y stages and LED. Nevertheless, we did not find any such relationship between DEB and clinical characteristics.
Conclusions: These findings provided evidence that the RSWA was associated with severity of illness in PD patients. The relationship is more closer than DEB.
