Session Title: Parkinson's disease: Non-motor symptoms

Objective: To analyze Parkinson’s disease (PD) motor manifestation and onset’s side influence on the spectrum of non-motor symptoms (NMS).
Background: Possibly the onset’s side and motor manifestation determine features and the prognosis of PD.
Methods: 796 patients with Parkinson’s disease (PwPD) are registered in movement disorders electronic database of the Siberian region. 260 non-demented PwPD were included (mean age: 66.4±5.6 years; PD mean duration: 6.8±5.6 years; H&Y stages 1–4, women:men=112:148) and devided into four groups by onset’s side and motor manifestation (homogeneous by gender, age, stage): I– 66 right-side, tremor (RT), II– 64 left-side, tremor (LT), III– 68 right-side, rigidity (RR), IV– 62 left-side, rigidity (LR). Clinical assessments were carried out using the UPDRS, H&Y Scale, MoCA-test, Beck depression inventory II, HADS, Apathy Scale, PD Sleep Scale, Epworth Sleepiness Scale, Questionnaire for Impulsive-Compulsive Disorders in PD-Rating Scale, PDQ-39.
Results: In I group major NMS were revealed: depression (66.67%), vivid dreams (62.12%), sleep behavior disorders (60.60%), cognitive impairment (54.54%), hallucinations (22.73%); in II– insomnia (54.69%), daytime sleepiness (23.44%); in III– depression (76.47%), restless legs (69.11%), cognitive impairment (39.70%), sexual difficulties (35.30%), sleep behavior disorders (29.40%); in IV– dysphagia (70.97%), urinary urgency (64.50%), insomnia (61.29%), sexual difficulties (45.16%), restless legs (43.55%), weight loss (29.03%), daytime sleepiness (27.40%). The onset’s side and motor manifestation determine the NMS presence and QoL in PwPD: better prognosis is in RT, in contrast to LR.
Conclusions: Onset’s side and motor manifestation may be used as a predictive indicator for the symptomology and PD prognosis allowing selective treatment strategies.
