Objective: To evaluate the relationship between fatigue and sleep disturbances in patients with Parkinson’s disease (PD).
Background: In patients with PD, fatigue is an important non-motor symptom, which has a negative impact on quality of life of sufferers. In a previous study performed in Japan showed that 43% of PD patients suffered from fatigue and fatigue was related to sleep disturbances as measured by PD sleep scale (PDSS) (Okuma et al, Mov Disord, 2009). In our previous study, there was a correlation between PDSS-2 total score and Parkinson Fatigue Scale (PFS) (Suzuki et al, J Neurol Sci, 2012). In this study, we performed the sub-analysis to see the relationship between sub-items of PDSS-2 and PFS.
Methods: A total of 93 patients with PD (age, 69.7±8.9 years) and 93 healthy controls (age, 69.6±10.2 years) from a single center study were analyzed (Suzuki et al, J Neurol Sci, 2012). The participants completed Parkinson Fatigue Scale (PFS), Epworth Sleepiness Scale (ESS), PDSS-2 and Beck Depression Inventory (BDI)-II. For PD patients, PD Questionnaire (PDQ-39) was used to evaluate quality of life. Fatigue was defined as PFS of 3.3 or greater.
Results: Fatigue was observed in 7.5% of healthy controls and 44.0% of PD patients. In the healthy control group, subjects with fatigue were older that those without fatigue, but there was no difference in PDSS-2, ESS and BDI-II scores. In the PD group, patients with fatigue had higher scores of ESS, BDI-II, PDQ-39, PDSS-2 and UPDRS part III than those without fatigue. The presence of fatigue was associated with PDSS-2 sub-item 2 (Difficulty falling asleep), item 10 (Pain in arms or legs), item 13 (Tremor on waking), item 14 (Tired and sleepy after waking in the morning), item 15 (Snoring or difficulties in breathing).
Conclusions: Our study results showed the significant relationship between fatigue and various aspects of PD-related nocturnal problems, highlighting the need for assessment of fatigue in patients with PD.
