Objective: To conduct a comparative analysis of remote digital assessment methods for evaluating speech, fluency, and cognitive function in individuals diagnosed with Progressive Supranuclear Palsy (PSP).
Background: PSP is a rare neurodegenerative disorder that clinically resembles other movement disorders like Parkinson’s disease (PD). As a result, PSP is frequently misdiagnosed, or diagnosed late in the disease course. A potential solution is a multimodal remote monitoring system using digital health technology to reliably and objectively capture PSP symptoms.
Method: Participants with PSP were recruited and monitored over a 12-month period. Clinical evaluations, including the PSP Rating Scale (PSPRS) and MoCA, were conducted every three months for a total of 5 assessments. Digital assessments of psychomotor skills, speech, and cognition were gathered using the BioDigit Home tablet from BioSensics, Newton, MA, USA. These included a digitized versions of the frontal assessment battery, digit span, and fluency tests.
Results: At present, the study comprises 18 individuals diagnosed with PSP[AP1] who have been enrolled and monitored for a duration of 3-12 months. Moderate effect sizes were observed in correlations between several parameters[AP2] of digital speech and cognitive assessment scores, when compared to the relevant clinical scores (e.g., bulbar subdomain scores of PSPRS and MoCA).
Conclusion: Multi-modal remote monitoring offers promise in assessing clinical progression in PSP, providing insights into speech, fluency, and cognitive function. We aim to develop a digital PSP Rating Scale for precise measurement in each of these domains. This will enable remote capture of relevant outcomes, improving access for patients with travel burden. Ultimately, our findings may enhance PSP clinical care and therapy development.
