Objective: In this observational survey, we report on a multicentre dataset addressing whether such vitals are being routinely assessed.
Background: Parkinson’s disease (PD), considered a syndrome (PS), is a progressive multi-system disorder, which requires pharmacological and non-pharmacological management to provide modern holistic bespoke personalised therapy. A recent dashboard of the “vital” measures of PD (Chaudhuri et al 2022) has been published based on expert opinion and patient preferences.
Method: Retrospective data extracted from Nonmotor International Longitudinal Study (NILS) database was included, as well as prospective data from clinics in the UK, Romania, Russia, Dubai, India, South Africa and Italy. We investigated, using observational surveys, the assessment for the Chaudhuri’s vitals, which consist of [1] motor, [2] nonmotor, [3] vision, oral and gut health, [4] bone health and falls, and [5] co-morbidities, co-medications, and impulse control disorders (ICDs).
Results: In this ongoing survey, we analyse data from February 2022 to December 2022 from 669 subjects thus far. A simple “yes”, “no” or “not applicable” format was used. Motor vitals (Hoehn and Yahr staging) was assessed in >90% of patients nonmotor vitals (NMS questionnaire score) in >50% of patients. However, vitals of vision (night vision 33%, driving 16%) gut (helicobacter pylori 2%, constipation screen 28%), and oral health was poorly assessed in clinic. Bone health (osteoporosis check/vitamin D supplementation) and falls (prevention strategies) was not documented and bone scans performed in only 21% across the sites. Specific advice on anticholinergic use and dopamine D3 agonist use was not clearly recorded.
Conclusion: This initial ongoing international pilot survey shows the lack of use of the domains included in the vitals dashboard and therefore, an important unmet need in clinical practice, which can be aided by practical application of the dashboard in clinics. Such a strategy will have implications on falls and fractures, levodopa absorption as well as safety regarding driving and vocations such as swimming in susceptible PD patients.
