Objective: We aimed to compare gait and parkinsonian features between PD patients, VP patients and PD patients with Vascular component (PDVC) using a smartphone-based motion sensors application
Background: Accurate differentiation between Parkinson disease (PD) and Vascular parkinsonism (VP) is not always possible based on clinical symptoms alone. It is even more difficult in PD patients with brain vascular lesions
Method: 16 PD, 12 PDVC and 13 VP patients were recruited. Gait was assessed using 10-M “timed-up-and-go” (TUG) test and gait stride Using “Mon4t Clinic” application with smartphone attached to the chest. Tapping speed was evaluated by pressing buttons in the application. Tremor was recorded with the smartphone laid on the palm
Results: Mean age of patients did not differ between groups. 10M TUG was significantly slower in VP compared to PD patients (56±31 vs. 29.9 ±8.6 seconds; p<0.01), however PDVC compared to PD was insignificantly slower (42±31 seconds). Step length was shorter in VP compared to PD patients (36±0.15 vs. 56±0.16 cm; p<0.01) and non-significant between PDVC and PD patients (51±0.16 cm, p=0.3). Rest tremor was detected in all groups. There was a tendency to more tremor asymmetry index in PD patients compared to VP and PDVC (5.4±12.2 vs 2.7±2.2 and 3.1±3 respectively; p=0.4). There was a tendency to slower tapping speed in VP compared to PD patients (13 vs 15 times per 20 seconds, respectively, p=0.1)
Conclusion: The “Mon4t Clinic” application is a useful tool to differentiate between PD, VP and PDVC. The 10M TUG test is the most sensitive parameter. Using this tool, we show that gait parameters of PDVC are more similar to PD than VP
