Objective: Completing the list of Hemichorea Hemiballismus apart from stroke and hyperglycaemic states.
Background: 61years old female presented with subacute onset abnormal irregular purposeless movements of one side upper and lower limbs.She responded to symptomatic treatment but within hospital stay developed same side hemiparesis.She also had past history of sensorineural hearing loss that responded to short course of oral steroids 18 months prior to current complaints.Brain imaging showed T2 hyper intense lesion in contralateral basal ganglia area with contrast uptake.Biopsy revealed presence of atypical lymphocytes suggestive of diffuse large B cell lymphoma and she was started on rituximab but could not survive and succumbed 3 month later to illness.

Conclusion: In conclusion, Acute and subacute presentation of Hemichorea and Hemiballismus can be lymphoma or malignancy apart from other more common etiology like stroke ,abscess an hyperglycaemic state.
