Objective: Assessment of incidence of insomnia in PSP, its connection with motor, neuropsychological and behavior disorders, evaluation of impact on the quality of life.
Background: Sleep disroders are common manifestations of Parkinson plus syndromes, which burdens the life-style of many patients.
Methods: The study included 124 patients with a diagnosis of PSP, mean age – 51,3± 0,8 years, stage – 2,46 ± 0,5, the duration of disease -6,5 ± 3,2 years. Neurological examination included Hoehn-Yahr scale, UPSPRS, Spielberger scale of anxiety, Parkinson’s disease Sleep Scale, Excessive Sleep Scale, Parkinson Fatigue Scale, Scale for Outcomes of Parkinson’s disease-Cognition, Lille Apathy Rating Scale, apathy SE Starkstein ASP, Parkinson’s disease Quality of Life questionnaire.
Results: Sleep disorders were observed in 38%, night awakenings with sleep fragmentation – in 86%, the early awakening – in 53% of patients. Higher frequency of sleep disturbances, nighttime awakenings were reported in women(p<0,001). There was a high trend of insomnia to increase with increasing severity and duration of PSP. Patients suffering from insomnia before motor stage of PSP (p <0,05) differed from the other groups of patients with a late age of onset of PSP. (p <0,05), low self-esteem of the quality of sleep, high frequency of nighttime awakenings, lower quality of sleep, higher severity of akathisia, non-pleasant dreams, nocturia, morning drowsiness, morning hypokinesia, depression.
Conclusions: Sleep disorders in patients with PSP are heterogeneous, partly due to the age factor, mobility and non-motor disorders. Based on the comprehensiveness of insomnia in PSP, effective therapy of patients with sleep disorders should to be individualized based on the nature of motor violations.
