Objective: We measured changes of retinal thickness, and compare them with clinical features and intracranial neurodegeneration in progressive supranuclear palsy (PSP) cohort.
Background: PSP has a variety of visual symptoms, which may be partly caused by structural abnormalities in the retina. The relationship between retinal structure changes and intracranial neurodegeneration was still unknown.
Method: A total of 19 patients with PSP were recruited. Participants underwent optical coherence tomography of peripapillary and macular, as well as brain 11C-2β-carbomethoxy-3β-(4-fluorophenyl) tropane (11C-CFT) and 18F-fluorodeoxyglucose (18F-FDG) PET. We compared peripapillary retinal nerve fiber layer (pRNFL) and average macular thickness with 22 age-matched healthy controls. False discovery rate was applied for multiple comparison adjustment. The correlations between retinal thickness changes and clinical severity, striatal dopamine transporter (DAT) availability and regional brain metabolism were also investigated.
Results: The pRNFL and macular thickness were significantly thinner in patients with PSP compared to controls. The superior sector of pRNFL showed a significantly negative relationship with the Unified Parkinson’s Disease Rating Scale part III score and Hoehn&Yahr staging scale score. A significantly negative correlation was found between average and outer inferior macula and duration of disease. Outer temporal macular thickness was significantly and positively correlated with CFT values in accumbens and caudate. Average, outer temporal and outer inferior macular thickness was significantly positively correlated with FDG values in cerebellum Crus I.
Conclusion: The pRNFL and macular thinning may be a candidate marker for monitoring motor symptoms and disease progression, and provide an in vivo indicator of intracranial neurodegeneration in PSP.
