Objective: To estimate prevalence of Parkinson’s Disease (PD) and Atypical Parkinsonisms and characterise the diagnosed population in a country with a development economy
Background: The Colombian Ministry of Health has implemented SISPRO, a national health registry gathering information from the National Health System, which covers approximately 95% of the Colombian Population. Registries also can present information about access to prompt and accurate neurological diagnosis.
Method: This is a descriptive epidemiological study in which we used the data collected by SISPRO to estimate prevalence and characterise patients diagnosed with PD and Atypical Parkinsonisms (Progressive supranuclear palsy (PSP) and Multiple system atrophy (MSA) in people over 50 years old in Colombia registered from January 2016 to December 2021. Criteria for diagnosis are not explicitly addressed in individual cases as diagnosis were made by the National Health Services
Results: National records report that the number of reported diagnoses of PD has been growing since 2016, in which 16,019 cases were reported and almost doubled to 29,970 cases reported in 2021. The estimated national prevalence for PD in people over 50 years old for 2021 is 6,54 per 1000 subjects (based on the total Colombian population over than 50 years old), being more frequent in males (56,4% cases). PD has its higher prevalence in people aged between 80-84 years old and is more frequently reported in Bogotá and least reported in Vaupés. For 2016, only 53 cases of PSP were reported nationally, most of them reported in people within 75 to 79 years old. No cases of MSA were reported in 2016. In 2021, 113 cases of PSP were reported, most of them in Bogota and in people between 70-74 years old. Same is the case for MSA, in which 2 cases were reported, all of them in Antioquia.
Conclusion: Colombia reported a lower prevalence of PD and Atypical Parkinsonisms compared with global and regional registries. Data suggest that subdiagnosis exists in Colombia, especially in terms of Atypical Parkinsonisms, and that diagnosis, as well as registries are centralized to big cities. These results might question access to a prompt diagnosis in vulnerable areas of the country.
