Category: Surgical Therapy: Parkinson's Disease
Objective: The aim of this study is to compare the clinical and pharmacological response of bilateral STN DBS in patients with monogenic forms of PD versus a control group of patients with idiopathic PD not carrying mutations.
Background: Bilateral subthalamic nucleus (STN) deep brain stimulation (DBS) is the surgical treatment of choice in Parkinson’s disease (PD). Patient selection and genetic background may modify the response to surgical treatment.
Method: A retrospective analysis of 23 patients carrying genetic mutations (8 PRKN and 15 LRRK2) and 74 patients with idiopathic PD was performed. UPDRS scale scores (II, III and IV) and levodopa equivalent dose before and after surgery (at 6 and 12 months) were compared.
Results: In the statistical analysis of these variables, no statistically significant differences were found between the groups studied. The results indicate that the clinical and pharmacological benefit of bilateral STN DBS is similar in patients carrying mutations in the PRKN and LRRK2 genes and patients with idiopathic PD.
Conclusion: In our study the clinical and pharmacological benefit of bilateral STN DBS does not differ between patients with idiopathic PD and patients carrying mutations in PRKN AND LRRK2 genes.
