Objective: To validate the Thai translation of the MDS-sponsored revision of the UPDRS (MDS-UPDRS).
Background: The MDS-UPDRS was published in 2008 to assess different domains of Parkinson’s disease (PD). Twenty-one Official Translations of the MDS-UPDRS have been approved. Here we present the result of the clinimetric assessments of the Thai version of the MDS-UPDRS (MDS-UPDRS-THAI).
Method: The English version of the MDS-UPDRS (MDS-UPDRS-ENG) was translated into Thai and then back-translated into English. The translated version underwent 2 rounds of cognitive pretesting in 16 patients and 3 raters to assess the ease of comprehension, ease of use and comfort with the scale. Then, it underwent large clinimetric testing.
For the primary analysis of the Thai data, confirmatory factor analysis (CFA) was conducted to determine if the factor structure for the MDS-UPDRS-ENG could be confirmed in data collected using the Thai translation. As a secondary analysis, an exploratory factor analysis (EFA) was conducted for the MDS-UPDRS-THAI Parts I-IV to explore the underlying factor structure without the constraint of a pre-specified factor structure.
Results: MDS-UPDRS-THAI was validated in 354 PD patients in 6 institutions. The CFA results are based on the Comparative Fit Index (CFI) for all four Parts of the MDS-UPDRS-THAI. The CFI, in comparison with the English-language factor structure, was 0.93 or greater. In the EFA, where variability from sample to sample is expected, we identified isolated item difference of factor structure between the Thai and English version of MDS-UPDRS. Indeed, several items had cross loading on multiple factors in the Thai scale.
Conclusion: The overall factor structure of the Thai version was consistent with that of the English version based on the high CFIs for all the four parts of the MDS-UPDRS in the CFA (all CFI >= 0.90). The Thai scale was confirmed to share a common factor structure with the English scale. Hence, this version has been designated as the official Thai version of the MDS-UPDRS.
