Objective: To determine the relation between anxiety and depression with tremors in patients with PD so that it can be possible to detect the symptoms as early as possible
Background: Anxiety and depression are non motor symptoms found in Parkinson’s disease (PD) and go undetected because of overlapping motor symptoms while tremor is a typical motor symptom experienced by 75% of patients with PD
Method: This study used a cross- sectional design. The subjects were PD patients at the neurologica polyclinic at Haji Adam Malik Hospital Medan, North Sumatra Hospital and Kesdam Bukit Barisan Hospital during 17 September 2020- 20 December 2020 who met the inclusion criteria. Anxiety assesment used the Hamilton Anxiety Rating Scale (HARS) Questionnaire, depression with the Beck Depression Index (BDI) and tremor used Unified Parkinson’s Disease Rating Scale (UPDRS) III (Specific items for tremors 20-21). The Data was analysed using the Spearman correlation test.
Results: The 28 subjects who met the inclusion criteria, 17 patients were male (60.7%), the mean age of the subjects was 64.96 ± 7.73 years. Most education level is higher education, 9 retirees (32.1%) and 12 Bataknese (42.9%). The most severity level of PD based on H&Y on stage 1 was 11 people (39.3%), the average length of time consuming L-Dopa was 4.84 years (SD = 3.43). The highest level of anxiety and depression is no anxiety and severe 8 persons (28,6%) and no depression 13 Persons (46,4%). There’s a significant relation between HARS and tremor score (r = 0.847; p <0.001) and BDI and tremor score(r =0.737; p <0.001).
Conclusion: There’s a significant relation between anxiety and depression with tremor in PD patients. The more anxiety and depression, the more severe the tremors will be.
