Objective: The aim of our study was to reveal the relationship between motor and muscle activity шт terms of plantar pressure distribution PPD) in Parkinson’s disease under the influence of transcranial magnetic stimulation of the basal ganglia of the brain.
Background: Transcranial magnetic stimulation (TCMS) is a method based on stimulating the neurons of the brain with an alternating magnetic field without invasive intervention.The study of inhibitory processes in the brain using TCMS is possible due to the ability of TCMS to excite inhibitory neurons in the cortex. There are several methods for recording inhibitory responses using TCMS. Methodologically simplest method consists in measuring duration of suppression of spontaneous muscle activity after presentation of magnetic stimulus (excitation by electromagnetic induction).
Method: We used 3D motion and force measurement technologies for biomechanics evaluation of 127 Parkinson’s disease patients, men and women from 52 to 79 years old. Control group consisted of 111somatically and neurolgically healthy donors by the same age. Plantar pressure distribution was evaluated using ZEBRIS system before and after TCMS course. The therapeutic extended version of the Neuro-MS / D magnetic stimulator meets all the clinical requirements of specialists in the field of rhythmic transcranial magnetic stimulation (rTCMS).
Results: Stabilization of the foot load, improvement of pressure and the time of maximum strength of the toes and heels, depending on the phase of support, were noted during the treatment.We observed also normalization of other neurological functions, reduce symptoms of parkinsonism such as akinesia, rigidity and tremor. Motor manifestations were achieved in the form of a reaction of certain skeletal muscles. The associative zones were activated.
Conclusion: PMD could be considered for early PD diagnosis method and for measure for different therapy modalities including TCMS. The combined use of relatively new technical means in neurology should help in the diagnosis and treatment of PD.
