Objective: To analyze the effects of Gandouling tablets(GDL) on the tone and stiffness of the biceps brachii muscle in Wilson’s disease (WD) patients, and the correlation between spasms and muscular rigidity.
Background: Wilson disease (WD) was first systematically described by Kinnear Wilson in 1912. It is an autosomal recessive neurological disorder that is characterized by chronic and progressive copper ion metabolic dysfunction . Recent clinical evidence suggests that DMPS significantly lowers copper levels and improves spasticity and quality of life of WD patients, without causing adverse reactions. However, Dystonia is the most common complication of Wilson disease (WD) and can seriously affect the quality of life of patients and increases the socio-economic burden. At present, there are few reports on the objective evaluation and treatment of WD dystonia.Therefore, it is vital to develop new evaluation and therapeutic strategies for WD dystonia.
Method: Sixty patients with myotonic WD were recruited and randomly divided into the treatment and control groups (n=30 each). Fifty-seven patients completed the study. All participants were given regular copper discharge therapy with sodium dimercapto sulphonate (DMPS). GDL were administered to the treatment group, whereas the control group received a placebo. Following treatment for 1, 2, 3 and 4 weeks, the muscle tone and stiffness of the patients were measured with the MyotonPRO devise using the MultiScan pattern of five measurements. The spasmodic state was assessed by neurologists using Ashworth Scale (AS)
Results: Bicep muscle tension, stiffness and spasticity scores did not change significantly after 1 week of GDL administration compared to that in the placebo control group (P>0.05), but decreased at the 2 week time-point (P<0.05) and remained significantly lower till the end of 4 weeks (P<0.05). Spearmen rank correlation showed a statistically significant positive correlation between muscle tension, stiffness and AS score in all participants (P<0.05)
Conclusion: GDL can reduce biceps brachii tension, stiffness and spasticity in cerebral WD patients. Myoton PRO device can objectively evaluate the viscoelastic properties of muscle, which are associated with spasticity.
