Objective: The aim of the study was to determine reliability and validity of the ST in PD.
Background: Parkinson’s disease (PD) is affected hand functions negatively. However, validity of any measurement tool has not been determined in clinical trials. The Squarest Test (ST) is a simple, administered and understandable easily test. In the Multiple Sclerosis population, the ST has high discriminatory power and shows highly significant correlation with Nine Hole Peg Test (NHPT).
Methods: Fifty-seven idiopathic PD patients and fifth healthy people were included in the study. At baseline, the ST, Unified Parkinson’s disease Rating Scale (UPDRS), Hoehn and Yahr (H&Y) Scale was applied to the PS group when the patients were “ON” period. The ST was repeated after seven days from the first application when PD group were “on” period to evaluate its reliability. To determine responsiveness of minimal clinically important difference (MCID) that described as a change between 2.5 and 5.2 on UPDRS motor examination (UPDRS-3), the ST and UPDRS-3 was repeated when approaching “OFF” period in PD group.
Results: Test-retest reliability of the ST was excellent in the PD group for right and left hand (ICC2,1=0.955, ICC2,1=0.935, respectively). Discriminatory power of the ST between groups was excellent. The ROC area under the curve was 0.936 (AUC=0.936). Discriminative optimal cut-off point between groups was determined as 114 dots (sensitivity 94%, specificity 84%). The ST was well correlated with UPDRS-total and UPDRS-3 scores and was moderately correlated with H&Y Scale (r=-0.690, r=-0.637, r=-0.598, p<0.001, respectively). The ST score difference was significant between before MCID and after MCID (p<0.001). The effect size was large (Effect Size=0.89). Change between the ST and UPDRS-3 scores was strongly correlated (r=0.820, p<0.001).
Conclusions: The ST is a valid, reliable and responsive to MCID method for measuring hand functions in PD.
