Objective: To assess the effects of sleep disorders on the quality of life in PD patients.
Background: Parkinson’s disease is a progressive neurodegenerative disease. The non-motor symptom(NMS) is said to be more disabling than the motor features. sleep disorders are found in 98% of PD patients, they are to result from the disease process, medications or nocturnal disturbances. A large-scale study involving 1,072 PD patients showed that the non-motor symptoms with the largest negative impact on quality of life(QoL). Every domain of QOL is reported to be affected by sleep disorders. This study seeks to find out the effect of sleep disorders on the quality of life of PD in Ibadan.
Method: This was a descriptive cross-sectional study conducted at the University College Hospital, Ibadan. Eighty- four consecutive consenting adults with PD were recruited. Patient’s information including socio-demographic data, medical and social history, sleep hygiene, PD history and medications were recorded.
PD patients were diagnosed according to specified diagnostic criteria and rating scales of PD staging (Hoehn &Yahr), cognitive functioning (Mini Mental State Examination (MMSE), sleep disorders (Parkinson’s disease sleep scale (PDSS), Epworth Sleepiness Scale(ESS), REM behavioural disorder (REMBD) screening questionnaire, STOP BANG for Obstructive sleep apneas (OSA)and quality of life (PD Quality of Life Questionnaire(PDQOL-39) were applied on all PD patients. Statistical significance was set at p<0.05.
Results: Participants without sleep disorders had significantly lower PDQOL scores in all domains with lower scores denoting a better quality of life. Median total PDQOL scores among PD patients without sleep disorders was 17.4 (IQR: 11.4 – 31.1) as opposed to 38.4 (IQR: 27.7 – 47.1) in those with sleep disorders, p value <0.001. The median mobility score was 18.8 (IQR: 5 – 47.5) in those without sleep disorders versus 51.3 (IQR: 27.5 – 77.5) in those with sleep disorders, p-value: 0.002. In the cognition domain, participants without sleep disorders scored 18.8 (IQR: 6.3 – 31.3) versus the 43.8 (IQR: 31.3 – 62.5) scored by those with sleep disorders, p-value <0.001.
Conclusion: Sleep disorders in PD negatively impact on the health related quality of life, sleep disorders should be actively sought for in PD patients and treated.
