Objective: To report intercountry differences in non-motor symptoms in patients with advanced Parkinson’s Disease (APD) vs non-APD.
Background: There is no standard criteria for the diagnosis of APD and local practices can vary between countries, making it difficult to anticipate treatment outcomes as Parkinson’s disease (PD) progresses. OBSERVE-PD examined disease characteristics in patients with APD.
Method: OBSERVE-PD was an observational, cross-sectional, multi-country study. The study included a total of 2615 PD patients from 128 movement disorder centers in 18 countries. Investigator assessment was used to categorize patients as APD or non-APD. Non-motor symptoms were assessed using the Non-motor Symptom Scale (NMSS) during a single study visit. Descriptive statistics were used to analyze results.
Results: Mean total NMSS score varied by country, but was consistently higher for APD patients than non-APD patients, reflecting an increased non-motor symptom burden in APD patients (total NMSS score intercountry range, APD: 32.4 to 87.8; non-APD: 14.7 to 53.2) [table1]. More patients with “mild” NMSS scores had non-APD than APD (51.7% to 92.9% had non-APD), except in 4 countries (50.0% to 68.8% had APD) [table2]. Across countries, with the exception of Ireland, the majority of patients with “very severe” NMSS scores were categorized as APD (54.5% to 100% had APD) [table2]. Across all NMSS score domains (cardiovascular including falls; sleep/fatigue; mood/cognition; perceptual problems and hallucinations; attention and memory; gastrointestinal; urinary; sexual function; and miscellaneous), patients with APD generally had higher mean scores than did those with non-APD.
Conclusion: These findings confirm that overall non-motor symptom burden was worse for patients categorized as APD than those who were non-APD. Patients with APD had higher mean NMSS scores, and this trend was generally consistent across individual NMSS domains. More patients with “very severe” NMSS total scores had APD than non-APD. The findings were consistent over the participating 18 countries.
