Objective: To determine the prevalence of selected co-pathologies in patients with Lewy Body disorders, as well as their influence on disease expression.
Background: The presence of mixed pathologies in the ageing brain is increasingly recognized. Whilst the potential of these co-pathologies to influence disease expression is widely appreciated in Alzheimer’s disease, it is less well studied in other neurodegenerative conditions (such as Lewy Body disorders). We have undertaken a systematic review examining the prevalence and influence of selected co-pathologies in patients with Lewy body disorders (Dementia with Lewy Bodies, Parkinson’s disease with or without dementia, and REM Sleep Behavior Disorder). The systematic review was done in collaboration with the Canadian Consortium on Neurodegeneration in Aging (CCNA) Team 8.
Methods: Two electronic libraries (Medline and Embase) were systematically searched using a pre-specified strategy for relevant papers published between 1st January 1997 and March 2017. Additional papers were identified via citations and because they were known to the review authors. Only prospective cohorts, retrospective cohorts, case-controlled studies, cross-sectional studies and case series (with more than 10 Lewy Body disorder brains) were included. Duplicate publications were removed. The study population included individuals with a clinically-diagnosed and autopsy-confirmed Lewy Body disorder and at least one other pre-defined co-pathology (amyloid-beta pathology, tau pathology, vascular pathology, TDP-43 pathology, hippocampal sclerosis or agyrophilic grain pathology).
Results: The search yielded 12,261 papers (6327 in Medline and 5934 in Embase). Following title/abstract screening, 384 were deemed eligible for full text review, which is ongoing. Final data extraction will be completed and summarized by the presentation date.
Conclusions: The search to date has identified frequent co-pathology in patients across the spectrum of Lewy Body disorders. The influence of such co-pathologies on the course of disease will ultimately require prospective longitudinal studies.
