Session Title: Parkinson’s Disease: Clinical Trials, Pharmacology And Treatment

Objective: The aim of the study was to correct the dyskinesia of the extremities, to study the efficacy and safety of drug therapy in men suffering from Parkinson’s disease with overactive bladder.
Background: According to various authors, lower urinary tract symptoms(LUTS) occurs in 37 to 90% of patients with Parkinson’s disease( (PD). They basically have a neurogenic form of a overactive bladder (OB).
Methods: Thirty two men with PD were examined.The mean age was 71 + 12 years. Clinical neurological examination was carried out with the help of the following scales: Hen-Yar scale , UPDRS scale (part II + III). Severity of LUTS was assessed according to the IPSS scale,OB scale (Homma, 2003) and diary of urination. All men, along with the main neurological treatment recieved trospium chloride (TC) at a dose of 15-45 mg / day,in a daily 3-times oral dose and tamsulosin (T) at a dose of 0.4 mg in a daily oral intake during the month.If occured side effects the dose ТC decreased or treatment stopped.Assessment of the quality of life evaluated by KHQ (King’s Health Questionnaire).
Results: For all types of medical dyskinesias, amantadine was prescribed as a corrector, prescribed a single dose of levodopa and levodopa / carbidopa / entacapone.By taking TC at a dose of 15 mg / day with T for 10 days, patients noted an improvements: a decrease the frequency of urination by 20%, the number of urgent urges by 31% and an increase in bladder compliance(BC) by 18%. When the absence of side effects, the dosage of TC was increased to 45 mg / day; the results were evaluated after 3 weeks. The frequency of urination decreased by 35%, the number of urgent urges by 45% and BC increased by 25%. The emotional state of men improved, anxiety and irritability decreased (t = 2.44, p = 0.015) and the quality of sleep improved (t = 2.35, p = 0.019).
Conclusions: Early diagnostics of medicinal dyskinesias and individual correction of antiparkinetic therapy allows to improve the quality of life of patients and reduce the degree of their disability. Trospium chloride a therapeutic dose of 45 mg/day in combination with Tamsulosin 0.4mg day is a highly effective in treatment OB in patients with BP. This not only decreases symptoms of OB, but also significantly improves the quality of life of men.
