Objective: The purpose of our study was to determine whether atrophy of Superior cerebellar peduncles (SCP) on phase-weighted magnetic resonance imaging technique (PADRE) was useful to discriminate progressive supranuclear palsy (PSP) from Parkinson’s Disease (PD).
Background: The differential diagnosis between PSP and PD may be difficult on the basis of clinical symptoms and radiological images especially in early-stage. SCP atrophy may contribute to the diagnosis of PSP, despite of the insufficient diagnostic accuracy on conventional MRI. PADRE can enhance the myelin density, can delineate SCP and the decussation of SCP passing through midbrain clearly by myelin concentration of nerve fiber.
Methods: Axial PADRE images were obtained with a 3D multi-echo spoiled gradient echo sequence at a 3T MRI system. A reviewer was blinded to the clinical diagnoses, and measured the SCP width in the slice of decussation of SCP on each side using PADRE from 15 PSP patients and 28 PD. Comparison between PSP and PD was made using the Mann-Whitney U test. Receiver operating characteristic (ROC) curve analysis was used to assess the sensitivity and specificity for diagnosing PSP from PD.
Results: Mean SCP width on PADRE in PSP (1.1±0.4 mm) was significantly smaller than in PD (2.3±0.3 mm) (p<0.05). Areas under the curves was 0.978 (95% CI 0.944-1.000). At the cutoff value was 1.7mm, the diagnostic sensitivity and specificity were 98.5% and 93.3% in differentiating PSP from PD.
Conclusions: We demonstrated that SCP width on PADRE may yield high diagnostic accuracy for the discrimination of PSP from PD on an individual basis.
