Objective: Truncal and Our aim is to report unusual case of man who developed prominent extensor truncal and cervical dystonia induced by mirtazapine.
Background: Truncal and cervical dystonia may occur due to various etiologies including some drugs, but mirtazapine induced truncal and cervical dystonia is extremely rare.
Methods: Single case report and description of clinical characteristics.
Results: A 54-year-old korean man was referred to our neurologic department due to involuntary truncal and cervical movements. He presented relatively subacute-onset, repetitive truncal and cervical dystonic movements two months ago. His truncal dystonic symptoms had more aggravated in lying position than standing or sitting position, so his upper trunk and neck muscles were continually waving with prominent extensor muscle of trunk in lying position. He had intermittent shortness of breath and chest tightness without other medical problems. He had diagnosed panic disorder in previous hospital and he had taken some medications including mirtazapine. Other neurological examination, brain imaging and electrophysiological tests were normal. After mirtazapine was discontinued, his dystonic symptoms gradually improved and completely resolved two months later.
Conclusions: To our knowledge, this is the first case report of prominent extensor truncal and cervical dystonia induced by mirtazapine. As like this case, if there is acute or subacute-onset, not-fixed, continual waving truncal and cervical dystonia with psychiatric medications, it should be considered diagnosis of drug-induced truncal and cervical dystonia.
